Bartonella behandeling

Hoi allemaal.

Naast de Lyme en tig virussen die actief zijn, heb ik vanochtend ook te horen gekregen dat de Bartonella actief/positief is.

Welke behandeling , zowel regulier als natuurlijk kan het beste helpen bij Bartonella?

In Nederland hebben Lyme artsen onvoldoende kennis van Bartonella, onderzoek en testen op Bartonella en de behandeling van Bartonella.
Ik kom geen informatie tegen dat Lyme artsen in Lyme centra als Nijmegen Amsterdam Rotterdam testen op Bartonella. Co-infecties worden niet getest of worden niet belangrijk gevonden.
Het buitenland is veel verder op het gebied van co-infecties en Bartonella.

Bartonella

Bartonella henselae is a gram-negative rod-shaped bacterium. It is known as the pathogen of cat-scratch disease. This infectious disease is transmitted by scratching injuries caused by cats and is identified on the basis of lymph node swelling on the armpit, neck or groin region and often progresses in a benign manner. Patients with existing immune weaknesses tend to sicken more easily and for longer periods.

New studies have shown that ticks in endemic areas can be infected with Bartonella, therefore it is likely that tick bites may result in subsequent infections

Bacteria:

Risk factors:

bites by ticks, lice, sand flies, scratches by cats, immune suppression (children)

Complications:

endocarditis, bacilliary angiomatosis, retinitis, epilepsy, aseptic meningitis, hepatosplenomegalia, tonsillitis, cerebral arteritis, encephalitis

De kattenkrabziekte is een infectie met Bartonella henselae, veroorzaakt door een beet of een krab van een met B. henselae geïnfecteerde kat of (wellicht) door een besmette teek.

Vele onderzoeken uit omringende landen – en eerdere onderzoeken uit Nederland – komen echter tot andere conclusies en chronische lymepatiënten testen opvallend vaak positief voor Bartonella. Behalve B. henselae zijn inmiddels enkele tientallen andere Bartonella soorten bekend, waarvan sommige zoals B. bacilliformis (overgebracht door zandvliegen) en B. quintana (overgebracht door luizen) dodelijk kunnen zijn.

Welke van deze soorten in Nederland resp. in teken voorkomen en in hoeverre ze door een teek kunnen worden overgebracht op mensen is vaak onbekend; de meeste soorten zijn vooral voor vakantiegangers van belang. Bartonella is een intracellulaire bacterie die net als Borrelia allerlei mechanismen heeft om de afweer te omzeilen en manipuleren. De informatie op deze pagina betreft in eerste instantie alleen B. henselae.

De bacterie is ongevoelig voor de meeste antibiotica; voor Rifampicine en Co-trimoxazol lijkt de bacterie wél gevoelig te zijn. De aandoening kan echter ook chronisch worden in het bijzonder bij mensen met een verzwakt immuunsysteem. De infectie kan ook herleven, wat kan gebeuren na b.v. een onvoldoende lange antibioticakuur voor een andere infectie
Het is goed mogelijk dat voor de lymepatiënt de combinatie van deze infecties verslechterend werkt. Helaas is er nauwelijks onderzoek verricht naar het verloop van een B.henselae infectie in lymepatiënten.

Bartonella moet in Nederland veel serieuzer worden genomen. Dat zal waarschijnlijk nog wel een tijd(je) gaan duren ben ik bang.
Intussen is het aan ons de patiënten zelf om je goed te laten informeren en te gaan lezen over Bartonella. En met de Lyme artsen-behandelaars die wel op de hoogte zijn van Bartonella te overleggen over een behandeling van de Bartonella.

Er zijn verschillende mogelijkheden die afhankelijk zijn van belangrijke factoren als bijvoorbeeld; de ernst en het aantal klachten en schade die zijn ontstaan aan organen, zenuwstelsel en lichaamssystemen door Lyme andere virussen en Bartonella, hoe sterk/zwak het lichaam op het moment is, hoe sterk/zwak het immuunsysteem op het moment is, hoe sterk/zwak de organen en lichaamssystemen op het moment zijn om een behandeling natuurlijk of regulier met antibiotica aan te kunnen.
De behandelend Lyme arts kan dat het beste beoordelen.

Ik vraag me af of meerdere hier zowel de B henselae als de B quintana actief hebben.

For serious Bartonella infections, it is critical to use two antibiotics, each of which has good in vivo efficacy against Bartonella.

This is particularly important if gentamicin is one of the drugs in the regimen, because the gentamicin protection assay with red blood cells infected in vivo, as well as the in vitro erythrocyte cell culture model, document that bartonellae residing within erythrocytes are protected from gentamicin (90, 93). Thus, with our current knowledge, addition of another antibiotic with good in vivo activity against Bartonella is crucial, because the two antibiotics may eradicate the bacteria in different niches in the host.


B. HENSELAE:

The only prospective treatment trial, a double-blind, placebo-controlled study of azithromycin treatment of immunocompetent patients with uncomplicated CSD, was reported by Bass et al. (8). An 80% decrease in the initial lymph node volume was documented in 7 of 14 azithromycin-treated patients but in only 1 of 15 placebo-treated controls during the first 30 days of observation

B. QUINTANA

patients with chronic B. quintana bacteremia or trench fever should be treated with gentamicin (3 mg/kg i.v. once daily) for the first 14 days, in addition to doxycycline (200 mg daily) for 28 days (Table ?(Table6,6, recommendation AI) (36). Doxycycline could be given either as a single daily dose of 200 mg every 24 h or as a twice-daily dose of 100 mg every 12 h. Patients with chronic bacteremia should be carefully evaluated for endocarditis,

In dit artikel wordt steeds gentamicin genoemd in combinatie met andere ab's, afhankelijk van welke soort Bartonella infectie je hebt opgelopen.
En het artikel zegt ook dat de manier van behandelen te maken heeft met een acute of een chronische vorm van Bartonella.
