Increasing Evidence Points to Inflammation as Source of Nervous System Manifestations of Lyme Disease;

About 15% of patients with Lyme disease develop peripheral and central nervous system involvement, often accompanied by debilitating and painful symptoms. New research indicates that inflammation plays a causal role in the array of neurologic changes associated with Lyme disease, according to a study published in The American Journal of Pathology. The investigators at the Tulane National Primate Research Center and Louisiana State University Health Sciences Center also showed that the anti-inflammatory drug dexamethasone prevents many of these reactions.

“These results suggest that inflammation has a causal role in the pathogenesis of acute Lyme neuroborreliosis,”

Some patients with Lyme disease also show evidence of demyelinating neuropathy and slowing nerve conduction. Nerve conduction studies in motor and sensory nerves of the macaques showed that the Bb infection resulted in specific electrophysiological abnormalities (increased F wave latencies and chronodispersion) that could be prevented with dexamethasone.

Lyme neuroborreliosis (LNB), caused by the spirochete Borrelia burgdorferi, affects both the peripheral and the central nervous systems. Radiculitis or nerve root inflammation, which can cause pain, sensory loss, and weakness, is the most common manifestation of peripheral LNB in humans. We previously reported that rhesus monkeys infected with B. burgdorferi develop radiculitis as well as inflammation in the dorsal root ganglia (DRG), with elevated levels of neuronal and satellite glial cell apoptosis in the DRG. We hypothesized that B. burgdorferi induces inflammatory mediators in glial and neuronal cells and that this inflammatory milieu precipitates glial and neuronal apoptosis.

Central nervous system (CNS) vasculitis is inflammation of blood vessel walls in the brain or spine. (The brain and the spine make up the central nervous system.) CNS vasculitis often occurs in the following situations:

Accompanied by other autoimmune diseases such as systemic lupus erythematosus, dermatomyositis, and, rarely, rheumatoid arthritis;
Infection, such as viral or bacterial;
Systemic (affecting the whole body) vasculitic disorders (granulomatosis with polyangiitis (GPA), microscopic polyangiitis, Behçet's syndrome);

Het autonome zenuwstelsel wordt ook vaak aangetast in het geval van een Lyme-infectie:
Er is een terugkerende toename of afname van de polsslag , toegenomen transpiratie , veranderingen in huidtemperatuur, spierspanning, fluctuaties in bloeddruk en perifere vegetatieve stoornissen en stoornissen in de bloedsomloop, zoals de symptomen van de Raynaud .

Bij encefalitis, meningitis of myelitis wordt het centrale zenuwstelsel direct beïnvloed.
Het kan loopverstoringen , halfzijdige aandoeningen, spraakstoornissen, blaasdisfunctie of het begin van epilepsie omvatten .
In de context van chronische Lyme-ziekte blijven concentratieproblemen, slaapstoornissen, aandoeningen van het fijne motorische chronisch vermoeidheidssyndroom en hoofdpijn beschikbaar.

Although the meninges and nerve roots are more commonly involved during Lyme borreliosis, especially in Europe, numerous reports from Europe and the United States have documented the occurrence of acute and chronic manifestations in the brain or spinal cord (Ackermann et al. 1985; Klenk et al. 1985; Pfister et al. 1985; Reik et al. 1986; Kohlhepp et al. 1987; Behringer and Wirbatz 1987; Weder et al. 1987; Wokke et al. 1987a; Kohler et al. 1988; Halperin et al. 1989). The following chapter describes the clinical features and necessary investigations which help in the diagnosis of chronic central nervous system (CNS) involvement during Lyme borreliosis.

Het zenuwstelsel kan aangetast worden door verschillende infecties, bijvoorbeeld gordelroos, HIV, de ziekte van Lyme.

Abstract
A 61-year-old man presented to the emergency department in the summer with a right seventh cranial nerve lower motor neuron palsy and worsening paraesthesias for 6 weeks. He had debilitating pain at the scalp and spine. Prior work up was unrevealing. The patient resided in the upper Midwest region of the USA and worked outdoors, optimising the landscape for white tailed deer. Repeat cerebrospinal fluid testing revealed a lymphocytic pleocytosis and positive IgM Lyme serology. Brain MRI demonstrated enhancement of multiple cranial nerves bilaterally. He was diagnosed with early Lyme neuroborreliosis and treated with 28 days of intravenous ceftriaxone. While the painful meningoradiculitis, also known as Bannwarth syndrome, is more commonly seen in Europe, facial palsy is more frequently encountered in the USA. Clinical manifestations of neuroborreliosis are important to recognise as the classic presentation varies by geography and on occasion repeat serological testing may be necessary.
