We are pursuing FDA (US Federal Food and Drug Administration) approval here in the US and we will be pursuing approvals in Europe as well - ensuring that we can get our product out to Canada, the US and Europe. We're in the manufacturing process, making sure that we have validated the test in every possible way so that so that it is reliable, it gets FDA approved and we hope to be on the market by the end of this year or the beginning of next year. Als een arts een hypothese (een voorlopige stelling) heeft en van alles rond roept en de hypothese de hele (medische én wetenschappelijke) wereld wil gaan bewijzen is het toch vanzelfsprekend dat die arts er alles aan doet om dat medisch én wetenschappelijk met een Pilot Study én een opvolgende RCT te gaan bevestigen en bewijzen? En de Resultaten van de RCT te Publiceren in de gerenommeerde tijdschriften? Tot nu toe is dat nog steeds niet van de grond gekomen.

Geen study design, geen inclusie of exclusiecriteria, geen semi-objectieve manier om "beter" te definieren, geen poweranalyse, geen statistiek, geen medisch ethsche commissie, geen data safety monitoring board wat bij de combinatie van deze geneesmiddelen zeker geeist zou worden. De "special" waar dit verhaal onderdeel van is, bevat alleen dit artikel. Tot mijn verrassing wordt er geen "conflict of interest" gerapporteerd terwijl de auteurs de enigen zijn die patienten op deze manier behandelen en daar zeer goed voor betaald worden. Ik gun iedere Lymepatient een goede behandeling, maar deze cocktail van sterk werkende medicatie met serieuze bijwerkingen (van de cocktail zijn de bijwerkingen niet gerapporteerd) is zeer dubieus..

2017:..On May 22, Fred Verdult hosted Something Inside So Strong, an event in Amsterdam to raise awareness about Lyme disease, modeled on his annual HIV event, which fills a national theatre with over 800 people each year. Just as he refused to succumb to either the illness or the shame of it nearly 20 years ago when he was diagnosed with HIV, Verdult is not backing down when he’s told that Lyme -which he says has made him far more ill than HIV ever has- is no big deal, or all in his head..

Op 19 juni heeft Dr. Richard Horowitz op uitnodiging van de Lymevereniging in congrescentrum De Nieuwe Liefde in Amsterdam een masterclass gegeven voor *zo’n tachtig zorgverleners. Met inzichten over nieuwe persister-protocollen, o.a. met Dapsone, over behandeling met antibiotica en aanvullende middelen, over co-infecties, over zijn 16-punten plan ontwikkeld naar aanleiding van zijn decennialange ervaring met complexe lymepatiënten, en als de grand finale case studies van patiënten die na langdurige ziekte en een lange zoektocht met een gecombineerde aanpak toch opknapten. Fred Verdult, a longtime activist who was knighted for his work in The Netherlands, exemplifies the problem and progress. He takes one pill daily for HIV infection and has never been ill from AIDS. He has been disabled by, and treated with dozens of pills daily, for Lyme disease. “The contrast with Lyme couldn’t be greater,” he wrote in answer to my survey.

.. Mechanistic Context and Hypothesis
The authors proposed that persistent infection with Borrelia burgdorferi may contribute to neuroinflammatory processes linked to AD pathology, citing prior work demonstrating co-localization of spirochetes with amyloid plaques and tau in postmortem tissue.

Within this framework, DCT is hypothesized to target biofilm and persister forms of the organism, potentially reducing inflammatory triggers associated with tau phosphorylation and amyloid dynamics. Additional proposed mechanisms include modulation of inflammasome signaling and downstream neuroimmune pathways.

In the study discussion, the authors noted that, “This is the first case study showing that a short-term biofilm/persister drug regimen can potentially improve pathological triggers of neuroinflammation, including p-tau and amyloid-related biomarkers.”

However, they also acknowledged that AD is widely understood as a multifactorial condition, with infection representing only one of many proposed contributors, alongside genetic, metabolic, vascular, and environmental factors.

Clinical Context and Limitations
Despite the biologic signal observed, the findings are limited by the inherent constraints of a single case study. The authors emphasized that results are not generalizable and require validation in controlled clinical trials.

Current clinical guidelines from organizations such as the Infectious Diseases Society of America and the American Academy of Neurology do not recommend routine Lyme disease testing in patients with dementia, reflecting a lack of definitive causal evidence linking infection to Alzheimer disease progression.²

Additionally, prior randomized trials evaluating antimicrobial therapy in post-treatment Lyme disease syndrome have produced mixed or negative results, underscoring the complexity of attributing neurologic outcomes to infectious etiologies alone.³

The study also did not include longitudinal cognitive testing or imaging correlates, leaving uncertainty regarding the durability and clinical significance of the observed biomarker changes.

Broader Implications
While exploratory, the report contributes to an evolving body of literature examining the role of neuroinflammation and systemic contributors in neurodegenerative disease. Blood-based biomarkers such as p-tau217 have emerged as increasingly sensitive indicators of Alzheimer pathology, with strong associations to amyloid burden and future cognitive decline.

The authors call for larger, placebo-controlled studies to evaluate whether antimicrobial or anti-inflammatory strategies could have a role in specific patient subgroups, particularly those with overlapping infectious or inflammatory conditions.

Until such data are available, the findings are best viewed as hypothesis-generating rather than practice-changing, highlighting the need for continued investigation into the complex and heterogeneous drivers of AD.,

..We are going to start fundraising through my 501c3 the MSIDS Foundation (https://msidsfoundation.org/) to get the funding that was denied when I submitted my R34 NIH grant last year to prove in a gold-standard multicenter, randomized, placebo-controlled study, that dapsone combination therapy and the MSIDS model is a viable solution for those individuals suffering from CLD/PTLDS. We are updating the website now. Now that we have proof that many of our chronic LD patients are testing positive for these AD biomarkers *(see our prior Medical Detective Substack’s on the subject): and proving we can reverse AD biomarkers like p-tau 217 while improving beta amyloid ratios, we will incorporate these new biomarkers in our randomized, multicenter, placebo-controlled study. We would like to raise $250k to pay for a statistician and support the time involved from physicians/researchers. To date, universities that have expressed an interest in participating in this study are Dr David Putrino from Mt Sinai in NYC, Charles Chiu at UCSF and potentially the University of Arizona, associated with Dr Andrew Weil. If any other universities are interested, please contact me. Once we are satisfied with the statistics and all doctors, scientists and researchers agree on the design of the study, we will apply for government funding once more and attempt to get a two-arm trial to evaluate the diagnostics and therapeutics for both AD and CLD simultaneously. I will give Dr Dale Bredesen a call about the study and see if he and his team would like to participate. Geen study design, geen inclusie of exclusiecriteria, geen semi-objectieve manier om "beter" te definieren, geen poweranalyse, geen statistiek, geen medisch ethsche commissie, geen data safety monitoring board wat bij de combinatie van deze geneesmiddelen zeker geeist zou worden. De "special" waar dit verhaal onderdeel van is, bevat alleen dit artikel. Tot mijn verrassing wordt er geen "conflict of interest" gerapporteerd terwijl de auteurs de enigen zijn die patienten op deze manier behandelen en daar zeer goed voor betaald worden. Ik gun iedere Lymepatient een goede behandeling, maar deze cocktail van sterk werkende medicatie met serieuze bijwerkingen (van de cocktail zijn de bijwerkingen niet gerapporteerd) is zeer dubieus..

Additionally, prior randomized trials evaluating antimicrobial therapy in post-treatment Lyme disease syndrome have produced mixed or negative results, underscoring the complexity of attributing neurologic outcomes to infectious etiologies alone.³

The study also did not include longitudinal cognitive testing or imaging correlates, leaving uncertainty regarding the durability and clinical significance of the observed biomarker changes.

Broader Implications
While exploratory, the report contributes to an evolving body of literature examining the role of neuroinflammation and systemic contributors in neurodegenerative disease. Blood-based biomarkers such as p-tau217 have emerged as increasingly sensitive indicators of Alzheimer pathology, with strong associations to amyloid burden and future cognitive decline.

..Back in 2021, the Lyme community was buzzing about a potential new treatment called hygromycin A. It came out of Dr. Kim Lewis’s lab at Northeastern University, and the early news sounded genuinely thrilling.

Here was a compound that demonstrated selective killing against Borrelia, the Lyme pathogen, without harming the beneficial microbes in the gut. That’s important, because gut disruption has long been one of the biggest drawbacks of antibiotic treatment for Lyme.

Early studies looked promising. Hygromycin A cleared Lyme infection in lab mice, left their gut bacteria intact, and even showed potential for reducing Lyme in reservoir animals like field mice. With results like that, the next logical step was to see whether the compound could be developed into a treatment for people.

That’s where Flightpath Biosciences came in. The company licensed hygromycin A and began developing it into a drug known as FP100. Working closely with Dr. Lewis, Flightpath manufactured the compound, completed the required animal studies, and moved it into human testing. Phase 1 clinical trials began in 2024.

Recently, Flightpath CEO Matt Tindall shared an update on what the company learned from those trials—and the news wasn’t immediately what many Lyme patients had hoped for.

Hard lessons
“We learned hard but crucial lessons,” Tindall wrote in a letter to the Lyme community. “FP100 showed limited oral bioavailability in humans, meaning too little of the drug reached the bloodstream and tissues at levels needed for robust, reliable clinical benefit.”

In simpler terms: the medicine couldn’t consistently get to where it needed to go in the body for a long-enough duration, in an oral dosage form, to effectively treat Lyme disease.

Because of that, Flightpath is now going in a different direction. Still collaborating with Dr. Lewis, the team has shifted its focus to a new drug candidate named FP530 (forazemin).

According to Tindall, early findings suggest that FP530 clears Lyme pathogens in animal models, may circulate more effectively throughout the body, and potentially do so without impairing healthy gut bacteria. The company describes it as an even more promising drug candidate for patients with Lyme disease, though some supporting details have not yet been made public.

The good news is that FP530 is not far behind FP100 in development, and the company plans to begin a clinical trial later this year.

The bottom line? Creating a new Lyme drug is tough-probably tougher than many of us realize. But Flightpath Biosciences intends to keep at it. The company’s decision to pivot rather than give up reflects a commitment to getting this right.

Lessons Learned from FP100 (hygromycin A)
When we first advanced FP100, we did so because it represented a bold, targeted approach to Borrelia infections and aligned with our shared goal with patients and clinicians: a more precise, potentially more effective way to treat Lyme disease. In early clinical work, however, we learned hard but crucial lessons. FP100 showed limited oral bioavailability in humans, meaning too little of the drug reached the bloodstream and tissues at levels needed for robust, reliable clinical benefit. This raised concerns about whether it could reliably deliver the clinical benefits patients need. As patient advocates and developers, we must follow the data even when it means difficult decisions. FP100 did not meet the bar we believe Lyme patients deserve: consistent efficacy, ease of administration, and long-term viability as a broadly accessible medicine.

FP530 (forazemin): A New Direction
Our decision to prioritize FP530 (forazemin) is intended to focus our efforts on bringing the best potential product to patients. FP530 is a potentially much more bioavailable, bactericidal antibiotic designed to kill disease-relevant Borrelia and other pathogens more completely and efficiently, with a microbiome-sparing profile. In preclinical work and early clinical studies, FP530 has demonstrated strong systemic exposure, favorable pharmacology, and a potency profile that gives us hope it can achieve therapeutic levels where it matters most in the body.
As a next step, the company expects to enter a phase I healthy volunteer study to assess safety and tolerability with an oral formulation this year.

Lyme borreliosis (LB), caused by Borrelia burgdorferi sensu lato (Bbsl) through Ixodes tick bites, presents diverse clinical manifestations and may lead to persistent symptoms. This review summarizes current knowledge on the pathogen–host interactions and immune responses. Early infection can be influenced by tick saliva, which suppresses local host defense and promotes spirochete survival, and by pattern recognition receptors activating proinflammatory cascades. Bbsl employs a variety of immune evasion strategies, notably impairing antigen presentation—through disruption of MHC II and IFN-γ pathways—and continuously varying surface antigens to hinder long-lasting antibody formation. Autophagy plays a central role in modulating inflammation and T helper 17 adaptive immune responses, representing an underappreciated mechanism potentially influencing disease outcome. Adaptive immunity in LB is characterized by robust but often dysregulated humoral and cellular responses, with transient germinal centers and enduring IgM production contributing to incomplete pathogen clearance. Persistent immune defects include impaired long-term B cell memory, suppressed T cell activation, and ongoing immunosuppression after pathogen clearance. Similar patterns are observed in other postinfectious fatigue syndromes. Despite advances, gaps remain in understanding mechanisms of Bbsl persistence and the immunopathology underlying chronic disease, challenging diagnosis and therapy. Emerging molecular and cellular approaches offer new avenues to address immunity, diagnostics, and prevention. A multidisciplinary effort will be needed to improve long-term patient outcomes in the evolving epidemiology of LB..

..5 Current knowledge gaps and future directions
Despite decades of research, many critical questions about LB remain unanswered, and the accelerating challenges of persistent and chronic disease highlight the urgency of innovative solutions. Promising new directions center around unraveling the mechanisms underlying Bbsl persistence, with advanced molecular and imaging tools now making it possible to visualize tissue and cellular niches where spirochetes evade immune clearance [84]. High-throughput single-cell and multiplex profiling are set to reveal which cell types and microenvironments support ongoing infection and immunological dysfunction, enabling more targeted therapeutic strategies [55].

A renewed focus on restorative immunology holds great potential [7]. Repairing damaged germinal center architecture and reversing lymphoid tissue disorganization—perhaps by manipulating local cytokine milieus and chemokine pathways—could pave the way for more durable humoral memory and better resolution of infection [85]. Expansion in the field of autophagy also offers opportunities to dampen excessive inflammation and mitigate chronic symptoms, as new pharmacological and gene-editing approaches emerge [86].

Continued progress against antigenic variation remains vital for vaccine and diagnostic development. Cutting-edge protein engineering, epitope mapping, and AI-driven immune profiling are merging to outpace Bbsl's molecular shape shifting, promising broader and more reliable detection, and improved vaccine design that can account for diverse strains and immune responses [87-89].

Integrated “-omics” studies are beginning to yield predictive biomarkers for chronicity, offering hope that early risk stratification may soon be possible [90, 91]. This, alongside the rollout of multiplex rapid diagnostic tests and flexible laboratory platforms, is expected to transform both point-of-care management and long-term disease tracking in endemic areas [92, 93].

Genetic studies continue to elucidate susceptibility to disease and long-term complications, opening avenues for precision medicine and individualized prevention [94]. Functional genomics defining the QTL architecture of Bbsl-induced cytokine responses illustrates the impact of common genetic variants in immune and metabolic pathways. These variants program individual inflammatory set-points in LB [95]. As climate change and ecosystem alterations extend tick habitats, coordinated surveillance and prevention strategies will be crucial to reduce new infections and recurrences [96, 97].

Altogether, the future of LB research lies in bridging gaps across science, translational modeling, and patient-centered clinical interventions. Only through a multidisciplinary and global approach can the field respond to the evolving challenges of diagnosis, treatment, and prevention, ultimately restoring effective immunity and improving outcomes for individuals affected by this complex disease..

..Vandaag, 12 mei, is het wereldwijd ME/CVS Awareness Day. Een dag waarop aandacht wordt gevraagd voor mensen met ME/CVS: voor het moeten dragen van hun ziekte, de invloed daarvan op hun dagelijks leven en het gevecht voor betere diagnose en behandeling..

..Tot slot wordt ME/CVS ook nog vaak vernoemd naar de infectie waarmee het begon, zoals Q-koorts, de ziekte van Lyme of COVID-SARS-19. Hoewel de chronische symptomen niet kunnen worden onderscheiden van die van ‘andere’ patiënten met ME/CVS, wordt toch de term ME/CVS dan soms niet gebruikt. Dit geeft wel eens verwarring, bij zowel patiënten als niet-patiënten. Ook verwarrend is het feit dat wel duidelijk is wat de ziekte heeft veroorzaakt, maar desondanks niet goed wordt begrepen waarom het moeilijk herstelt.

Tegelijk is er de afgelopen jaren veel positief in beweging gekomen. ME/CVS wordt steeds gerichter onderzocht en vanuit een biomedisch perspectief onderzocht. Wetenschappers kijken onder meer naar het afweersysteem, de energiehuishouding, het zenuwstelsel, de bloedvaten en de manier waarop het lichaam reageert op inspanning. Ook buiten de wetenschap groeit de bekendheid met ME/CVS, mede door post-COVID. Steeds meer mensen zijn (helaas) bekend geworden met termen zoals PEM en POTS. Wat vroeger vaak moeilijk uit te leggen was, krijgt langzaam meer woorden, meer onderzoek en meer erkenning..

Dit zijn opnieuw eerlijke en duidelijke antwoorden van de wetenschappers & onderzoekers en daar ben ik persoonlijk blij mee:.

Hopelijk voor alle patiënten (mensen die langdurig ziek zijn geworden na infecties (bijvoorbeeld virus, bacterie)) en ook voor de Lyme patiënten in Nederland! De tijd zal het gaan leren!


Persoonlijk heb ik in de afgelopen tijd/jaren geleerd dat infectieziekten erg complexe ziekten zijn!
CMV (HHV-5) en EBV (HHV-4) waren in het verleden bij een ISO 15189 geaccrediteerde Medische Laboratorium in Nederland positief/doorgemaakte infectie. Van EBV ben ik in het verleden erg ziek geweest en ben ik er niet van hersteld en is
PEM (Post Exertional Malaise (PEM) is terugslag na inspanning) ontstaan en een blijvende beperking geworden. Op basis van het niet herstellen van EBV en de PEM is de diagnose ME gesteld. En Varicella Zoster Virus (VZV) (HHV-3) is meerdere keren positief geweest. En de ziekte van Lyme en de ernstige ziekte B12 tekort/B12 deficiëntie als gevolg van een absorptie-/opname probleem zijn daar bijgekomen.
Een huisarts die wél luisterde heeft mij verteld dat evenals de andere herpesvirussen CMV en EBV na een primo-infectie
latent in het lichaam aanwezig blijven en dat er veelvuldig reactivaties kunnen optreden. Dat zou vaak gebeuren bij een verminderde weerstand en opnieuw ziekte en klachten en of verergering van klachten kunnen veroorzaken!

Bij mij is intussen duidelijk geworden dat de ziekte van Lyme/chronische Lyme én de ziekte ME naast elkaar bestaan en is er bekend geworden wanneer welke ziekte is ontstaan & begonnen. Voor mij persoonlijk wordt er eindelijk steeds meer en meer duidelijk. Herkenning & Erkenning!!! En dat doet mij goed en geeft rust! Ik werd van een gezonde persoon die een actief en werkzaam leven had een persoon die door de jaren heen steeds zieker en zieker werd en op een gegeven moment huis/bank/bed gebonden raakte! Ziek worden heeft mij erg veel gekost qua gezondheid en op maatschappelijk- en sociaal gebied. Verbetering is het hoogst haalbare geweest en genezing is niet meer mogelijk geworden.

Ik heb herinfecties (nieuwe nimfen beten) met EM (rode kring) gehad. Verschillende huisartsen herkenden dit niet en namen het niet serieus.
Op basis van de teken en de nimfen beten, de ACA's, rashes en EM, de ziekte geschiedenis en het ziekteverloop heeft een nieuwe huisarts op een gegeven moment alsnog de klinische diagnose van de ziekte van Lyme gesteld en bevestigd. Maar dit was jaren te laat met alle gevolgen van dien!..

Blijf alert: het is belangrijk om goed op de hoogte te blijven van zogenoemde nieuwe spelers op de Lyme-markt. Er zullen steeds weer zogenoemde 'nieuwe testen' en 'nieuwe behandelingen' worden aangeboden!
De ziekte van Lyme en chronische Lyme zijn een verdienmodel geworden! En allerlei commerciële partijen én behandelaars in privé-klinieken, Ilads- en AVIG Lyme artsen, natuurartsen en therapeuten in Nederland of in het buitenland schromen er niet voor om over de rug van (érg) zieke mensen, Lyme patiënten of andere (zoekende, hoopvolle of wanhopige) zieke mensen veel geld te verdienen!

.. 2.4 Bloedonderzoek in buitenland of via privékliniek in buitenland
Bij 41% van de deelnemers is bloedonderzoek gedaan in het buitenland of in een privékliniek in Nederland. De uitslag was bij 39% van deze groep (gedeeltelijk) positief en bij 2% negatief (figuur 20)..

..2.5 Overig onderzoek
Bij 36% is ander onderzoek gedaan om Lyme of een andere tekenbeetziekte vast te stellen. Veelal ging het om een lumbaalpunctie (65%). 20% kreeg een PCR-test, 11% een huidbiopt en 8% een gewrichtspunctie (figuur 21). Bij 27% is een ander onderzoek gedaan. Hier worden verschillende onderzoeken genoemd, waaronder een MRI-scan. Daarnaast worden er onderzoeken genoemd die geen deel uitmaken van de reguliere, gevalideerde diagnostiek voor Lyme (zoals LTT test, Levend Bloed Analyse, FISH test en bioresonantie test). Deze zijn dus niet gebruikt om de diagnose te stellen..

De (nieuwe) Lyme patiënten of chronische Lyme patiënten of andere (zoekende, hoopvolle of wanhopige) zieke mensen in Nederland worden hiermee absoluut niet goed geholpen!!

Conclusie & wake-up call: Er wordt helaas véél onjuiste informatie verspreid op internet, fora, in blogs of in Lymegroepen. En daarom moeten patiënten beter leren om zich zélf ook goed in de materie te leren verdiepen én goed te leren lezen!!! En hetgeen dat wordt gepromoot & geadviseerd door anderen (patiëntenorganisaties, patiënten, 'activistische Lyme organisaties&groepen', Ilads Lyme&me/cvs artsen, AVIG Lyme artsen, natuurartsen, therapeuten en behandelaars in privé-klinieken) goed te verifiëren en daarmee het risico te vermijden om ergens (onwetend, in goed vertrouwen, naïef, 'blind') achter aan te gaan lopen of achter zogenoemde op een voetstuk geplaatste 'Lyme-helden' aan te gaan lopen.

Fabrikanten moeten door middel van een uitgebreidere validatie, wetenschappelijke onderbouwing én studies naar de klinische prestaties van hun producten aantonen dat hun product werkzaam en veilig is.
