Patiënten én patiëntenorganisaties, 'activistische Lyme organisaties&groepen', Ilads Lyme&me/cvs artsen, (AVIG) Lyme artsen, natuurartsen, therapeuten en behandelaars in privé-klinieken (wereldwijd) worden misleid!!!

In Lyme-land wordt er helaas (door sommige patiënten en door de Nederlandse Lyme ('activistische') patiëntenorganisaties) veel onjuiste informatie (o.a. over kostbare (experimentele) en niet-gevalideerde testen van commerciële laboratoria, kostbare (experimentele) behandelingen, kostbare holistische/alternatieve behandelingen en testen, etc.) gegeven.

Als geteste patiënt (volgens de uitslagen: Borreliose, 7 coinfecties, erg lage waarde Natural Killer Cells, erg lage waarde CD57 NK-cells
--> conclusie: Chronische Lyme) bij voorheen: BCA Duitsland/BCA Lab (InfectoLab) heden: ArminLabs (Duitsland) ben ik nog steeds 'not-amused' dat de cellulaire testen jarenlang zijn aangeraden door de Lymevereniging en Stichting Tekenbeetziekten. Ik vertrouwde er toen ook op dat de verstrekte informatie goed was! Helaas zijn mijn toenmalige (AVIG) Lyme arts (reguliere en complementaire werkwijze én samenwerking met BCA Duitsland (heden: ArminLabs Duitsland voorheen: BCA Duitsland/BCA Lab (InfectoLab)) van de Werkgroep Lyme en/of coinfecties en ik er ook ingetuind.

De gevolgen voor de (geteste en te testen) patiënten zijn: een onjuiste testuitslag, een gemiste diagnose, een verkeerd gestelde diagnose, een verkeerde voorgeschreven behandeling, veroorzaakte (blijvende) schade aan de gezondheid.

Met de kostbare en niet-gevalideerde TickPlex testen van Tezted (Finland) en of de kostbare experimentele en niet-gevalideerde Phelix Phage Borrelia test van Red Laboratories (België) en of de kostbare na validatie onbetrouwbaar geleken
LTT-EliSpot/iSpot Lyme testen van ArminLabs (Duitsland) en van samenwerkende partner Infectolab Americas (Verenigde Staten) kunnen géén diagnoses Lyme/chronische Lyme/neuroborreliose/Post-lymeziektesyndroomworden en of coinfecties (als bijvoorbeeld
B. miyamotoi/Relapsing Fever) worden gesteld! En kunnen er géén behandelingen mee worden gemonitord!!
En de testen en de 'positieve' uitslagen van de testen kunnen niet dienen als medisch én wetenschappelijk bewijs/onderbouwing van Case-studies!

..Onderzoeksresultaten - Subanalyses
Praktische problemen: cellulaire tests zijn foutgevoelig. De gevoeligheid na zes en twaalf weken is niet merkbaar verbeterd. De cellulaire tests presteren vergelijkbaar tussen:
- Patiënten met vroege lyme (erythema migrans) versus gedissemineerde lyme.
- Gezonde personen met een eerdere tekenbeet/Borrelia-antilichamen versus die zonder.

Combinatie van een antistoftest met een cellulaire test maakt deze niet beter.
Bij het testen van lymeziekte is in het algemeen de groep met gezonde personen (zonder lyme) veel groter dan de groep met
lymepatiënten. De onderzoekers stellen dat veel patiënten vals-positieve testresultaten krijgen met de door hen onderzochte testen.

Conclusie
De onderzochte cellulaire tests zijn ongeschikt voor gebruik, vanwege:
- lage gevoeligheid bij zowel vroege als gedissemineerde lyme.

The two-colour iSpot is a test that is currently commercially available in Germany, having been developed by the **company AID (also known as GenID). The iSpot is performed on-site at the facilities of AID/GenID in Strassberg, Germany. It uses 10 ml of heparinized blood.
***The iSpot uses an ***ELISPOT technique. The technique used for the present study is identical to the techniques used in daily clinical practice at AID/GenID’s facilities.
Whole blood samples are incubated with Borrelia lysate (B31) and a mix of recombinant Borrelia proteins (OspC, DbpB and OspA), as well as with a negative and a positive control. The production of two cytokines by T cells, namely IFN-γ and interleukin-2 (IL-2), is then assessed using the so-called ‘traffic light principle’. After incubation and development, wells that contain a high concentration of IFN-γ and a low concentration of IL-2 turn red, which according to the manufacturer indicates the presence of mainly effector T cells, which would be suggestive of an active infection.
A reversed concentration of IFN-γ and IL-2 is indicated by a green colour, suggesting a higher concentration of memory T cells, indicating a past infection. A balanced concentration of IFN-γ and IL-2 is said to indicate a latent infection. Test results are interpreted following the manufacturer’s instruction..

Deze test meet via antigenen het aantal CD57 lymfocyten, een bepaald soort bloedcellen die helpen om infecties op te ruimen. Met name chronische Lymepatiënten zouden een laag aantal CD57 cellen hebben en bij antibiotica behandeling en herstel van de ziekte zou dit aantal weer omhooggaan, tenminste volgens sommige ILADS-artsen. Ook bij diverse heel andere ziektes kan het aantal CD57 cellen echter verlaagd zijn en er zijn ook chronische Lyme patiënten die wél een normale of zelfs hoge CD57 score hebben. Het is dus niet terecht om de CD57 test een ‘Lyme test’ te noemen. De test heeft beperkte waarde voor Lyme diagnostiek en wordt in Nederland weinig gebruikt..

..Eerste Subsidie verstrekt
Het Lymefonds kon in 2020 de eerste subsidie verstrekken: de onderzoeksgroep van Yinyu Shan van de University of Leicester heeft €20.000 ontvangen. Het ontbreken van een goede test die al in een vroeg stadium kan vaststellen of iemand al dan niet een actieve lyme-infectie heeft, is een groot gemis en levert veel langdurige ziektegevallen op. Deze onderzoeksgroep werkt aan de ontwikkeling van een test voor de ziekte van Lyme en bartonella, een ziekteverwekker die je ook via teken kunt oplopen. Deze test werkt op een totaal andere manier dan de huidige tests, namelijk via bacteriofagen.

Voor onderzoek is €20.000 natuurlijk een bescheiden bijdrage. Omdat er aan Lyme wereldwijd zo weinig onderzoeksgeld wordt besteed, is de onderzoeksgroep uitermate blij met deze subsidie, ook omdat hieruit internationale waardering voor hun werk blijkt..

Conclusie & wake-up call: de gevolgen voor de (geteste en te testen) patiënten zijn: een onjuiste testuitslag, een gemiste diagnose, een verkeerd gestelde diagnose, een verkeerde voorgeschreven behandeling, veroorzaakte (blijvende) schade aan de gezondheid.

Veel mensen-patiënten zijn jarenlang(!) en worden tot op heden nog steeds misleid!!! Patiënten moeten beter leren om zich zélf ook goed in de materie te leren verdiepen en hetgeen dat wordt gepromoot & geadviseerd door anderen (patiëntenorganisaties, patiënten en of achterban; Bron viewtopic.php?f=5&t=2322&start=1140#p30443 , 'activistische Lyme organisaties&groepen', Ilads Lyme&me/cvs artsen, (AVIG) Lyme artsen, natuurartsen, therapeuten en behandelaars in privé-klinieken) goed te verifiëren en daarmee het risico te vermijden om ergens (onwetend, in goed vertrouwen, naïef, 'blind') achter aan te gaan lopen.
Gezondheid is een kostbaar bezit. Een verkeerd gestelde diagnose of een verkeerde experimentele behandeling kan érg schadelijk zijn voor de gezondheid. You want to give more money to the NIH that has ignored the science on chronic Lyme being a chronic persistent infection? See my prior FB post. Maybe I should ask for 13 million of their 130 million. That would pretty much give everyone the answers they need, as with that amount of funding, I could easily do a randomized, controlled trial on dapsone combination therapy and prove to everyone the role of MSIDS factors in chronic illness. That amount of money would fund a 5 arm trial. One traditional therapy, Plaquenil and Mino. One arm with classic DDDCT and HDDCT. One arm with a placebo, using all of the same treatment, minus dapsone. One arm just pulsing with HDDDCT every 4-6 weeks.. One arm pulsing, with a placebo, which is not dapsone, every 4-6 weeks.

This type of a study might even give us new approaches to help the Lyme community.

The National Academy of Sciences wants to call my version of Lyme-MSIDS, Lyme IACI. See below. Wow. Is it surprising? Of course not. This disease has been called a lot of things. In Australia, they call it DSCATT. Disease complex associated with ticks. Everyone wants to create another alphabet soup for this disease. Why do I think this is a problem? Because my model has been validated to work and help patients improve their health. Why does it therefore make more sense, from my highly biased opinion, to call it Lyme-MSIDS or non-Lyme MSIDS (for example, Bartonella and multiple factors keeping them ill)?

There are 6 broad categories of inflammation which can cause 10 downstream effects in the MSIDS map. That is the essence of the MSIDS model that helps diagnose and treat CLD. And it works for a broad range of other chronic diseases also, including long COVID. In fact, we published that all 16 factors on the MSIDS map have been found in long COVID:..

..So even if I have published 9 articles on dapsone combination therapy, clearly showing that Borrelia is a 'persister bacteria under biofilms' which responds to persister/biofilm drug regimens and pulsing, and diagnosing and treating MSIDS factors...and even if Tulane showed that rifampin and dapsone clears Lyme in the animal model, curing Lyme, none of this is strong enough to have governmental organizations admit that we have answers. Here is why:
Its more convenient for state and governmental agencies to deny that we have answers, and they feel they can justify their denial, because we don't yet have a randomized controlled trial on dapsone combination therapy. I'm speaking to an organization tomorrow in NC who helps get grants approved and funded. Wish me luck.

You say tomato, and I say tomato, and you say Lyme-IACI and I say Lyme-MSIDS. And although words shouldn't make a difference, they do. It is Lyme-MSIDS making Lyme patients sick. How do I know? I've treated 13,000 over the last 40 years. I have published what I have found in the medical literature. All of this for the Lyme community, who was desperate to have answers.

So have all the governmental committees you want. I have served at the Federal and State level. I did not see any major progress no matter how much I contributed, no matter how much I published. It appears it is only a RCT that will prove to the world that I have discovered answers. And likely, because some organizations did not discover it and in fact, denied what I have found, I wouldn't at all be surprised that when the RCT shows it was persister bacteria, under biofilm, who respond to pulses, and that there are up to 16 factors making people ill, then will likely they will call it something else. Not call it "Chronic Lyme disease and MSIDS". Or "Chronic Lyme disease, Chronic Babesia, and Chronic Bartonella and MSIDS"

Op 19 juni heeft Dr. Richard Horowitz op uitnodiging van de Lymevereniging in congrescentrum De Nieuwe Liefde in Amsterdam een masterclass gegeven voor *zo’n tachtig zorgverleners. Het Nederlands Lymeziekte-expertisecentrum is een samenwerkingsverband van het Amsterdam UMC, Lymevereniging Radboudumc in Nijmegen en de RIVM. Er is maar weinig informatie voor handen over de therapie die Hanaen krijgt. Veel kan het expertise-centrum er ook niet over vertellen. Het enige dat de deskundigen van het Amsterdam UMC erover kunnen zeggen is 'dat de SOT-therapie een experimentele therapie is zonder wetenschappelijke basis'..

Er wordt na een mediatraining bij Stichting Lymefonds/het Lymefonds door de patiënt in een Radio uitzending een ogenschijnlijk zelfverzekerd interview gegeven maar verificatie van informatie leert dat de patiënt allerlei onjuistheden vertelt.

..Can Oligonucleotide Infusions Really Fix Lyme, Cancer, and Other Diseases?
- U.S. clinics market oligonucleotide therapy for a wide range of conditions, without much evidence

Clinics across the U.S. are claiming to treat cancer, Lyme disease, and other bacterial and viral infections with "supportive oligonucleotide therapy (SOT)" -- a treatment lacking both data and FDA approval.

The name piggybacks off the success of more than a dozen oligonucleotide therapies approved by the FDA; Bron https://www.ncbi.nlm.nih.gov/pmc/articles/PMC10085713/ over the last quarter-century, including treatments for Duchenne muscular dystrophy, spinal muscular atrophy, and other rare diseases.

Many of the clinics are run by physicians or nurse practitioners, but some also have ties to naturopathic doctors. They lure patients with the promise of personalized treatment for difficult-to-fix conditions.

"Once this is in your body, it's almost magical," Connie Casebolt, MD, MPH, said in a YouTube video posted by Forum Health,
a network of integrative medicine clinics. "It can actually cause existing viruses and bacteria and Lyme organisms to die. It also prevents replication. So it's just a really great remedy, and we're seeing some really great results."

But there's no evidence that SOT works, save for two small and limited papers published by the Greek research team that appears to be the key supplier of tailor-made SOTs to U.S. clinics.

Mark Kay, MD, PhD, professor of genetics and pediatrics at Stanford University in California, said it sounded like a "scam."

"It really upsets me when I see this, because these things aren't benign. They do cause side effects," Kay told MedPage Today. "If you treat enough people, someone may have irreversible problems at some point, and none of this is doing any good."

What Is SOT?
Oligonucleotides are short strands of DNA or RNA that can play a role in the regulation of gene expression. Two main types of oligonucleotide drugs on the U.S. market are antisense oligonucleotides (ASOs) and short interfering RNAs (siRNAs).

Essentially, ASOs and siRNAs work by binding to much larger messenger RNA to regulate gene expression and ultimately manipulate the production of proteins, whether to increase them or shut them off.

FDA-approved ASOs include mipomersen (Kynamro) for homozygous familial hypercholesterolemia; eteplirsen (Exondys 51), golodirsen (Vyondys 53), viltolarsen (Viltepso), and casimersen (Amondys 45) for Duchenne muscular dystrophy; and nusinersen (Spinraza) for spinal muscular atrophy. siRNAs include patisiran (Onpattro) and inotersen (Tegsedi) for hereditary transthyretin amyloidosis.

Each of those compounds went through clinical trials and received FDA approval, unlike the SOT treatments being pitched by U.S. clinics.

In her video, Casebolt describes how patients first have a blood test to determine which pathogens they have, and then a blood sample is sent to the lab in Greece, which prepares the SOT based on those microbes. The tailor-made oligonucleotides then reportedly target bacterial genes involved in replication.

Casebolt says the "overall effects can be astonishing," and cited the case of a patient with Parkinson's disease who, after treatment with SOT, could play the violin again for the first time in 6 years.

Clayton Bell, MD, who works at a Forum Health clinic in Asheville, North Carolina, told MedPage Today that he has taken 10 SOT treatments for symptoms of tick-borne bacterial infections, and believes it has helped him.

He said he understands that clinicians may be "skeptical" because the treatments don't have "robust, clinical, double-blind, placebo-controlled trials" behind them; nor do they have FDA approval (though he said they don't need FDA approval because they're regulated as human cellular and tissue-based products, "like a PRP [platelet rich plasma] or like a stem cell").

"I've found it clinically to work. Not every time, but no medicine does," he said. "But when you use it judiciously, in the correct context with the right patient and the right support, I have found it to be very effective in a high percentage of cases."

Forum Health is hardly the only clinic offering SOT. A Google search yielded more than a dozen clinics -- in California (Los Angeles, San Clemente, and San Diego), Florida, Indiana (Carmel and Valparaiso), Kansas, Nevada, New Jersey, New York (Clifton Park and Manhattan), Pennsylvania, Tennessee, and Utah (Holladay and Greater Salt Lake City).

Most of these clinics cite the Research Genetic Cancer Center (RGCC), based in Greece, as the place where the SOT is made and then shipped back to the U.S. for intravenous infusion.

**Ioannis Papasotiriou, MD, PhD, is the founder of RGCC, which advertises "personalized cancer testing" on its website.

Papasotiriou is the corresponding author on the only two papers on SOT found in PubMed. The first, published in March 2022, is a preliminary study in 95 patients with cancer who received SOT. It reports that the majority of patients had a "positive" response to treatment, based on a composite of follow-up assessments for tumor response, but there was no control group.

The second paper, published in December 2022, concluded that SOT reduced the amount of DNA of Borrelia burgdorferi, a bacteria that causes Lyme disease, and other viruses in the blood of 115 patients.

It's not clear exactly what type of oligonucleotide is made for the therapy. The Lyme disease trial describes a siRNA, while an RGCC patient guide; Bron https://www.ipscell.com/wp-content/uplo ... .01.23.pdf describes an "antisense therapy."

RGCC has not returned a MedPage Today request for comment as of press time.

"We don't even know exactly what they're making," said Kay of Stanford. "siRNAs are different from ASOs but can accomplish some of the same things. They can knock down a gene product that you want to knock down."

However, it's not clear that either technology would work in bacterial cells. "As far as I know, there hasn't been any success in treating infectious diseases" like Lyme disease, Kay said. "Molecules would have to get into the bacterial cells to kill them, because you're targeting bacterial genes. As far as I know, this hasn't been accomplished yet."

Also, delivery has long been a challenge with oligonucleotides, which can be rapidly destroyed by the immune system, Kay said. They may be eliminated before they can get where they need to go.

Finally, he added, "if this was so simple, I think companies would have jumped on this to do appropriate preclinical and clinical trials."

Subject to FDA Regulation?
Paul Knoepfler, PhD, a stem cell biologist at the University of California Davis, who blogs about stem cells, sees parallels between the clinics giving patients SOTs and the stem cell clinics that the FDA has long been trying to regulate.

Stem cell products given out at clinics across the country have been linked to infections and severe adverse events, including hospitalizations. In 2021, the FDA mandated that all such treatments would need to be regulated as drug products -- not as human cellular and tissue-based products as they had long claimed -- or would require an investigational new drug (IND) application to be in place before being given to patients.

Yet many products continue to be sold, unregulated and without substantial clinical trial evidence to support them.

"I'm especially concerned about the cancer side of this as I don't see any good evidence to support that SOT would work for cancers and there could be many risks," Knoepfler told MedPage Today in an email. He pointed to an RGCC patient guide that "instructs cancer patients to stop taking some drugs like cytotoxic therapies, which are traditional cancer therapies, a pause that could pose risks."

Knoepfler believes that any oligonucleotide therapy -- even one that may be based on a patient's own blood samples -- would need to be regulated as a drug product by the FDA.

The agency has not returned a MedPage Today request for comment as of press time.

"Just because the FDA has approved a few other specific antisense or other related therapies doesn't mean that SOT would get approval," Knoepfler wrote on his blog. "It would depend on the data."..

Taking everything into consideration in terms of the preliminary data, SOT might be beneficial for patients with Lyme disease or infections by viruses, such as EBV and HSV. SOT is safe and specific for every target and can be used in many cases where a few drugs cannot be used. More samples need to be tested to implement SOT in the clinical routine; however, the preliminary inferential statistical data are encouraging for these infections..

De SOT-behandeling, waar Hanaen de hoop op heeft gevestigd, doodt de infecties stuk voor stuk. De Enschedese moet dus achttien behandelingen ondergaan. “Over twee weken krijg ik de zesde. Het slaat aan. Daar ben ik heel blij mee.” De weg naar verbetering is een heel ingewikkelde. Het onderzoek naar bestrijding van de specifieke injectie heeft plaats in Amerika, het medicijn in de vorm van een infuus wordt gemaakt in Griekenland en de behandeling is in Polen. “Het medicijn wordt dus specifiek voor mij gemaakt." Volgens Hanaen slaat de behandeling in Polen aan en nemen de bestreden infecties af. Ik durf weer te dromen, maar het blijft spannend.”..

Expertisecentrum
Het Nederlands Lymeziekte-expertisecentrum is een samenwerkingsverband van het Amsterdam UMC, Lymevereniging Radboudumc in Nijmegen en de RIVM. Er is maar weinig informatie voor handen over de therapie die Hanaen krijgt. Veel kan het expertise-centrum er ook niet over vertellen. Het enige dat de deskundigen van het Amsterdam UMC erover kunnen zeggen is 'dat de SOT-therapie een experimentele therapie is zonder wetenschappelijke basis'..

Wanhoop, hoop, onwetendheid en naïviteit kunnen een valkuil zijn en kan maken dat sommige mensen niet alle beschikbare informaties goed willen of kunnen gaan lezen en dat niet goed gaan verifiëren bij het maken van een keuze voor behandelingen (kostbare, experimentele, niet medisch en wetenschappelijk onderbouwde behandelingen). RGCC en de commerciële SOT privé-klinieken in het buitenland worden slapend rijk over de rug van zieke mensen!!!

..Here are the major problems with the RGCC paper.
1). Research Bias. The study was funded and conducted by RGCC who financially benefits by the way they interpret the data and publish the data.
2). There is No Control Group. The research does not include a control group of people given a placebo. Therfore we cannot tell what the placebo effect of benefit is in this study.
3). PCR-Determined Germ Load in Lyme is Prone to Error. The authors claim their data show after two treatments there are decreased DNA copies of Lyme in those studied. However, they used a PCR method to detect copies of Lyme DNA. It is well known that this method misses Lyme disease 70% of the time when Lyme is present. Quantitative PCR detection of DNA copies in Lyme is not reliable.
4). Statistical Hocus Pokus. To show positive result, the authors engage in statistical hokus pokus. They should have shared information showing actual decrease germ load for individuals treated so we can evaluate their claim.
5). Decreased DNA Copies May Not Equal Improved Symptoms. A useful endpoint in a Lyme treatment is that a patient feels better—that their health improves. The authors did not collect or publish health data on those treated.
6). Very Small Sample Size. We cannot make any generalizations about a treatment working based on a sample size of 28 people.

Besides the RGCC study, there is no other published research on tick-borne infections and SOT. And the RGCC study is terribly flawed. Thus, it is fair to say there is no credible research supporting SOT for Lyme or other tick-borne infections.

Profits over Science
It is worrisome that
- RGCC finally got around to conducting research to see if their therapy works years after they started profiting from their product and
- numerous clinicians are charging excessive amounts for this treatment without any credible scientific research showing that it actually works..

..We need better testing for tick-borne infections to track disease burden geographically and to provide an early diagnosis for these conditions. We need better awareness of these infections, both in patients and in medical communities. The clinical manifestations of these tick-borne diseases need better characterization, and we need publications in the medical literature to educate those who, knowingly or unknowingly, are evaluating such patients. The chronicity of these conditions needs to be better described in the medical literature and we need more publications on these conditions. We encourage research that addresses better diagnosis and treatment strategies for tick-borne diseases.

We invite submissions on the epidemiology of tick-borne diseases, as the true burden of diseases is underestimated. We invite submissions on new diagnostic tests for these infections, as current testing is insensitive, and we invite submissions on descriptions of clinical cases, case studies, and case series of these infections to better understand the range of diseases that tick-borne infections present. We invite pathogenesis studies that better characterize interactions of these infections with their hosts.
