Abstract
Neurological complications of Lyme disease include meningitis, encephalitis, dementia, and, rarely, parkinsonism. We present a case of striatonigral degeneration, a form of multiple system atrophy, in Lyme-associated parkinsonism. A 63-year-old man presented with erythema migrans rash, joint pains, and tremors. Serum and cerebrospinal fluid antibodies and polymerase chain reaction for Borrelia burgdorferi were positive. Clinical parkinsonism was diagnosed by several neurologists. Despite treatment, the patient continued to decline, with progressive disability, cognitive dysfunction, rigidity, and pulmonary failure. At autopsy, the brain showed mild basal ganglia atrophy and substantia nigra depigmentation, with extensive striatal and substantia nigral neuronal loss and astrogliosis. No Lewy bodies were identified; however, ubiquitin-positive glial cytoplasmic inclusions were identified in striatal and nigral oligodendroglia. There were no perivascular or meningeal infiltrates, the classic findings of neuroborreliosis. To our knowledge, this is the first report of striatonigral degeneration in a patient with B burgdorferi infection of the central nervous system and clinical Lyme-associated parkinsonism.

Alzheimer’s Disease: Assessing the Role of Spirochetes, Biofilms, the Immune System, and Amyloid-β with Regard to Potential Treatment and Prevention

Where spirochetes have been found in the brains of Alzheimer’s disease (AD), it may be considered an infectious disease; this is the first and most important consideration [1, 2]. It is also a chronic disease, a biofilm-associated disease, [3] and an autoimmune disease [4]. Further, it is a debilitating disease, a socially-destructive disease, an exceedingly expensive disease, and, lastly, a deadly disease [5]. This review will focus on the biofilm portion of the disorder as well as the autoimmune response. It will also touch on some rational therapeutic concepts for this most irrational of diseases.

Cystic borrelia in Alzheimer’s disease and in non-dementia neuroborreliosis

Background: A cystic form for Borrelia burgdoferi (Bb) was initially reported in 1988 in an autopsy study of Alzheimer’s disease tissue obtained from Dr. George Glenner’s brain bank. Cystic profiles were documented with silver stains, and with Murine Monoclonal antibodies to a Flagellin Epitope specific for Bb and B. hermseii [H9724 from Dr Alan Barbour]. Objective(s): Fresh frozen hippocampus tissues from Alzheimer’s disease cases provided by the Harvard University McLean Hospital brain bank were cultured in BSK M medium to attempt to grow spirochetes in vitro.
Methods: Triturated fresh hippocampus was cultured in vitro in sterile BSK M liquid media at 24 degrees C for one year. Darkfield microscopy examination and Acridine orange staining with epifluorescence microscopy was completed. Detection of specific Flagellin DNA sequences from ORF BBO147 using a Molecular Beacon DNA probe was used to measure incremental increases in Borrelia specific Flagellin DNA in cultures, as compared with the original fresh tissue retained uncultured DNA Extracts. Results: Cystic borrelia structures were recovered from in vitro cultures of fresh Alzheimer disease hippocampus tissue. Incremental increases in Borrelia burgdorferi flagellin B DNA were documented in cultured tissues.
Conclusions: A subset of Alzheimer’s disease is related to chronic Neuroborreliosis in the human host

Association between chronic periodontitis and the risk of Alzheimer's disease: a retrospective, population-based, matched-cohort study.

Abstract

BACKGROUND: Although recent short-term cross-sectional studies have revealed that chronic periodontitis (CP) may be a risk factor for increased cognitive impairment in patients with Alzheimer's disease (AD), systematic reviews and long-term longitudinal studies have provided less clear evidence regarding the relationship between CP and AD. Therefore, we conducted a retrospective cohort study using the National Health Insurance Research Database (NHIRD) of Taiwan to determine whether patients with CP are at increased risk of developing AD.

METHODS: We conducted a retrospective matched-cohort study using the NHIRD of Taiwan. We identified 9291 patients newly diagnosed with CP between 1997 and 2004. A total of 18,672 patients without CP were matched to the patient cohort according to sex, age, index year, co-morbidity and urbanisation level. Cox proportional hazards regression analyses were performed to evaluate the subsequent risk of AD.

RESULTS: Patients with CP had a higher prevalence of hyperlipidaemia, depression, traumatic brain injury and co-morbidities, as well as higher urbanisation levels, than those in the unexposed cohort (all p < 0.01). At the final follow-up, totals of 115 (1.24%) and 208 (1.11%) individuals in the CP exposed and unexposed groups, respectively, had developed AD. Patients with 10 years of CP exposure exhibited a higher risk of developing AD than unexposed groups (adjusted HR 1.707, 95% CI 1.152-2.528, p = 0.0077).

CONCLUSIONS: Our findings demonstrate that 10-year CP exposure was associated with a 1.707-fold increase in the risk of developing AD. These findings highlight the need to prevent progression of periodontal disease and promote healthcare service at the national level.

The advantage we have in 2019 is that DNA FISH (fluorescence in situ hybridization) stains that we now use are 99.99% specific and give us fluorescent images of the spiral bacteria in the thin sections of Alzheimer’s patients’ brains. In other words, we both see and identify the pathogen in the brain sections simultaneously with 99.99% accuracy.

Het aantal mensen met dementie gaat verdubbelen;
Het aantal mensen met dementie in Nederland groeit in een rap tempo. Er zijn momenteel 290.000 mensen met dementie en in 2040 zijn dat er naar verwachting ruim een half miljoen. Dat is bijna een verdubbeling.

..Case presentation
This patient died with severe dementia at age 68. His past medical history was positive for erythema migrans in perineal skin in his mid-fifties followed by persistent arthralgias, joint stiffness, autonomic dysfunction with hyperhidrosis, ‘transient global amnesia’ and pronounced fatigue from age 56-60. He retired from medical practice.
Dementia began at age 61 with cognitive and memory deficiencies, confirmed by neuropsychiatric testing. From 2003-2006 he developed dysarthria, agraphia, muscle spasms of the legs, photophobia and hyperacusis. Parenteral combination antibiotic therapy-pulsed with oral antibiotics was administered without durable curative effect from 2007-2009. He developed psychosis and died in 2010 in the seventh year of his dementing illness..

Heeft de diagnose " dement" gekregen op een gegeven moment.
Want de chronische vorm van lyme bestaat niet


Welke antibioticum en voor hoe lang weet ik niet, wie weet had ie andere ab moeten hebben, maar daar gaat dit stuk niet over.

A.MacDonald opereert niet als medicus.
