..Van deze patiëntengroep is uitgebreide informatie verzameld over de ernst van ziekte en kwaliteit van leven. In het nieuwe onderzoek zullen de ervaringen van patiënten met de door hen gevolgde behandelingen, zowel regulier als complementair, in kaart worden gebracht. Ook worden het beloop van de klachten op lange termijn en de impact van de ziekte op de arbeidsgeschiktheid onderzocht.
Uniek aan dit onderzoek is de participatie van patiënten als initiatiefnemer, ontwerper en uitvoerder van het onderzoek.

De initiatiefnemers van het onderzoeksvoorstel ‘Lyme legt levens plat’ hebben op donderdag 7 maart in Utrecht de ZonMw Medische Inspiratorprijs 2019 ontvangen. De Inspiratorprijs is de publieksprijs voor de meest inspirerende samenwerking tussen onderzoekers en patiënten. Het omvat een bedrag van €100.000, waarmee de initiatiefnemers dit PLEASE5+ onderzoek naar de langetermijneffecten van chronische Lymeziekte op de kwaliteit van leven, deelname aan de maatschappij en op arbeid kunnen uitvoeren..

Doel van het onderzoek is inzicht te krijgen in de kwaliteit van leven van chronisch lymepatiënten evenals in hun arbeidsparticipatie en de effecten van behandelingen die patiënten zelf hebben gezocht.
In januari en november 2023 zijn er bijeenkomsten van de onderzoeksgroep geweest. Analyses van de focusgroepen en interviews zijn gaande. Voorbereidingen voor een extra survey onder de achterban van de Lymevereniging en Stichting Tekenbeetziekten zijn eveneens gaande.
De eerste resultaten worden verwacht medio 2025..

Voor de **Randomized Controlled Trial (RCT) (is klinisch onderzoek en wetenschappelijk onderzoek een (cohort) study met verschillende patiëntengroepen én een controle groep) zijn de ambities erg hoog en de tijd zal leren of er een RCT opgezet kan worden en of de resultaten hetzelfde gaan bevestigen als allemaal is genoemd. Er is véél geld (5 miljoen dollar?!) voor nodig en de voorbereiding en het onderzoek zullen een aantal jaren gaan duren.

..8 maart 2023: Lyme treatment protocols are not always covered by insurers. This is why a RCT of dapsone combination therapy is essential. Appx 50% go into long term remission after an 8 week oral, generic protocol if all MSIDS factors have been addressed. Once I have more information about the success of our recent Lyme-Bart protocols, I will design a study and get input from the major Lyme organizations. I mentioned in jest this weekend during the LivLyme conference, that the major Lyme organizations need to consider giving Johns Hopkins center 5 million dollars to do the dapsone study. Its really not a joke. They have the largest PTLDS patient group and a successful study there would help move the needle towards proving the success of recent biofilm/persister regimens..

It is starkly different to the article published recently by Dr Adriana Marques. Isn't it interesting how two completely different conclusions can be reached by scientists quoting medical literature? How do you know which is true?
Clinical experience is ultimately the realm where truth is discovered and proved. And there is no doubt, with all of the different strains of Borrelia, and associated co-infections like Bartonella which can contribute to immune deficiency, that testing is unreliable; and the fact that both Borrelia and Bartonella are biofilm/persister bacteria, we see significant improvements in clinical symptoms using biofilm/persister drug regimens like dapsone combination therapy.

In June 2019, Kris Newby is publishing a book entitled “Bitten”, the secret history of Lyme Disease and biological weapons, in which she said to have interviewed Willy Burgdorfer, the discoverer of the disease's causative agent (171). Kris Newby explains that she watched a recording of Willy Burgdorfer, who died in 2014, in which he recounts working on biological weapons for the US army during the Cold War at Rocky Mountain Laboratories in Hamilton, Montana. His tasks included breeding fleas, ticks, mosquitoes and other blood-sucking insects or arthropods and infecting them with pathogens capable of transmitting disease to humans. As Willy Burgdorfer, before his death, gave part of his lab archives to Kris Newby, all what she wrote is proven. A few weeks after the publication of her book, the Chamber of Representatives voted in July 2019 a resolution asking for an investigative commission on the Pentagone activity in this field. Unfortunately, a senior official of the Ministry of Defense blocked the project.

If we look at history, a Nazi veterinary researcher, Erich Traub, worked in Germany during World War II to develop bioweapons. He studied on human prisoners several vector borne diseases, including Borrelia. Arrested by the Soviets in his laboratory on Riems Island, he was later exfiltrated to the USA in 1949, as part of the US government's Operation Paperclip. The goal of this program was to exploit the German scientific knowledge in the context of the Cold War leading to rivalry with the Soviet Union. Erich Traub had to help developing the “Plum Island animal disease center”. This center was located on Plum Island, some sixteen kilometers from the town of Lyme (Connecticut). This is what Kris Newby's book says: “Shortly before his death, Willy was filmed saying that he thought the epidemic of tick-borne diseases that had broken out around Lyme, Connecticut, had been caused by the spread of biological weapons. It was a stunning admission, but it could explain why Lyme disease is so difficult to diagnose and treat, and why the epidemic is spreading so far and so fast.” We learn from Willy Burgdorfer’s archives that in the Lyme area, at the very beginning of the epidemic, were observed many cases of infection with a newly man-made Rickettsia, Rickettsia helvetica, initially described in the archives as “l’agent Suisse” (“the Swiss agent”). Willy Burgdorfer could write his notes in French. This newly described bacterium was in fact made from a recombination of genes from Swiss Rickettsia and Rickettsia from the Rocky Mountains. Many cases of babesiosis were also reported in the region at that time. By reading these archives, we discover that the so-called Lyme epidemic, due to a single Borrelia species, was in fact a mixture of crypto-infections. Despite the denial of authorities, it has been proven that the army sprayed ticks marked with carbon 14 on: Montpelier (Vermont) in August 1966, September 1967, August 1968 and 1969, Newport News (Virginia) in September 1967 and August 1967, Mill Canyon (Utah) in August 1966 and July 1967. These elements raise two questions. Is there a connection with the growing epidemic of Lyme disease (ticks and micro-organisms possibly modified, more resistant and with increased infectious potential)? Do these military experiments explain the omerta on Lyme disease, given the potentially considerable responsibilities of the scientists and the states involved? (Appendix 2 of Kris Newby's book Bitten) (171, 172). There are also financial implications. Chronic Lyme disease can be expensive (173, 174). Conversely, we need to think about the fact that a cured Lyme disease patient (with inexpensive treatments) is a patient lost for expensive drugs, such as anti-TNF, which could be prescribed for the rest of his life. Indeed, as we have seen, this disease can mimic many other conditions, notably autoimmune ones. It is clear that, on a financial point of view, pharmaceutical companies do not favor the recognition of chronic Lyme disease..

..Is de bacterie in een lab gemaakt?
Er wordt wel eens gezegd dat de Borrelia bacterie die de ziekte van Lyme veroorzaakt in de jaren '40-'50 van de vorige eeuw ontstaan of gemaakt zou zijn bij Duits of Amerikaans biowapen onderzoek, of dat deze bacterie slechts sinds kort in teken voorkomt. Dit is allebei onjuist.

Op basis van o.a. genetisch onderzoek denkt men dat de Borrelia familie zeker 60 miljoen jaar oud is en in nauwe samenhang met bepaalde teken en diverse gastheren (o.a. reptielen, vogels en zoogdieren) geëvolueerd is. Er bestaan vele Borrelia varianten die over een groot deel van de wereld verspreid zijn en waarvan de meeste op basis van genetisch onderzoek al zeer oud moeten zijn. Via onderzoek van teken afkomstig van museumdieren weten we zeker dat de Borrelia bacterie ook al meer dan een eeuw geleden voorkwam in teken, zowel in Amerika als Europa..

Mary Beth Pfeiffer explains what tests are on the horizon, and why we should have hope. And when you read the amount of money that has been allocated for Lyme research in the past, compared to other diseases, it is pitiful.
Yet clarification is necessary. I have never in 41 years of clinical practice had difficulty establishing the diagnosis of chronic Lyme *(I don't see a lot of early Lyme these days). By taking a proper clinical history, doing a differential diagnosis, evaluating the patient for migratory pain (the hallmark symptom of Lyme disease) and using our validated symptom questionnaire, along with looking for evidence of other TBDs, we have always been able to find Borrelia (and associated co-infections). 1 oktober 2023:..Helaas heeft dit weinig met wetenschap te maken. Geen study design, geen inclusie of exclusiecriteria, geen semi-objectieve manier om "beter" te definieren, geen poweranalyse, geen statistiek, geen medisch ethsche commissie, geen data safety monitoring board wat bij de combinatie van deze geneesmiddelen zeker geeist zou worden. De "special" waar dit verhaal onderdeel van is, bevat alleen dit artikel. Tot mijn verrassing wordt er geen "conflict of interest" gerapporteerd terwijl de auteurs de enigen zijn die patienten op deze manier behandelen en daar zeer goed voor betaald worden. Ik gun iedere Lymepatient een goede behandeling, maar deze cocktail van sterk werkende medicatie met serieuze bijwerkingen (van de cocktail zijn de bijwerkingen niet gerapporteerd) is zeer dubieus..

..Zicht op langetermijnbeloop en afweer bij ziekte van Lyme
Het promotieonderzoek van aios Interne Geneeskunde Hedwig Vrijmoeth toont dat langdurige klachten 5% vaker voorkomen na de ziekte van Lyme dan in de algemene bevolking. Vrijmoeth promoveerde maandag 27 januari jl. aan de Radboud Universiteit in Nijmegen.

De ziekte van Lyme is goed te behandelen met antibiotica, maar sommige mensen houden na behandeling langdurig ernstige klachten, zoals vermoeidheid, pijn of concentratieproblemen. Er is nog veel onbekend over de oorzaak hiervan. Dit onderzoek, onder 1200 mensen die behandeld werden voor Lyme-ziekte, laat zien dat langdurige klachten 5% vaker voorkomen na Lyme-ziekte dan in de algemene bevolking.

The tests required to properly diagnose patients are not available in Australia, but they are available overseas, the committee heard.

They are not approved in Australia, but they should be, said Dr Leona Gilbert, an adjunct professor in cellular molecular biology who is the CEO of Tezted Ltd, a company that’s developing diagnostic tests, as well as Lyme Disease Association of Australia Scientific Advisory Committee chair.

The TGA uses ISO accreditations 13485 as a standard for tests, and NATA has accredited international labs by signing an international recognition agreement regarding the ISO 15189 standard, she said.

“That means NATA should have accepted and allowed these accredited overseas labs performing approved test kits,” said Dr Gilbert. “How we can do better is acknowledging these agreements; that actually there is good testing available. However, it’s not being employed in this country. There is Australian utility… and there are novel, innovative diagnostics like AI to be used on data from patients.”..

..Onderzoeksresultaten - Subanalyses
Praktische problemen: cellulaire tests zijn foutgevoelig. De gevoeligheid na zes en twaalf weken is niet merkbaar verbeterd. De cellulaire tests presteren vergelijkbaar tussen:
- Patiënten met vroege lyme (erythema migrans) versus gedissemineerde lyme.
- Gezonde personen met een eerdere tekenbeet/Borrelia-antilichamen versus die zonder.

Combinatie van een antistoftest met een cellulaire test maakt deze niet beter.
Bij het testen van lymeziekte is in het algemeen de groep met gezonde personen (zonder lyme) veel groter dan de groep met
lymepatiënten. De onderzoekers stellen dat veel patiënten vals-positieve testresultaten krijgen met de door hen onderzochte testen.

Conclusie
De onderzochte cellulaire tests zijn ongeschikt voor gebruik, vanwege:
- lage gevoeligheid bij zowel vroege als gedissemineerde lyme.
- lage specificiteit..

The two-colour iSpot is a test that is currently commercially available in Germany, having been developed by the **company AID (also known as GenID). The iSpot is performed on-site at the facilities of AID/GenID in Strassberg, Germany. It uses 10 ml of heparinized blood.
***The iSpot uses an ***ELISPOT technique. The technique used for the present study is identical to the techniques used in daily clinical practice at AID/GenID’s facilities.
Whole blood samples are incubated with Borrelia lysate (B31) and a mix of recombinant Borrelia proteins (OspC, DbpB and OspA), as well as with a negative and a positive control. The production of two cytokines by T cells, namely IFN-γ and interleukin-2 (IL-2), is then assessed using the so-called ‘traffic light principle’. After incubation and development, wells that contain a high concentration of IFN-γ and a low concentration of IL-2 turn red, which according to the manufacturer indicates the presence of mainly effector T cells, which would be suggestive of an active infection.
A reversed concentration of IFN-γ and IL-2 is indicated by a green colour, suggesting a higher concentration of memory T cells, indicating a past infection. A balanced concentration of IFN-γ and IL-2 is said to indicate a latent infection. Test results are interpreted following the manufacturer’s instruction..

..Het meten van het aantal CD57 lymfocyten
Deze test meet via antigenen het aantal CD57 lymfocyten, een bepaald soort bloedcellen die helpen om infecties op te ruimen. Met name chronische Lymepatiënten zouden een laag aantal CD57 cellen hebben en bij antibiotica behandeling en herstel van de ziekte zou dit aantal weer omhooggaan, tenminste volgens sommige ILADS-artsen. Ook bij diverse heel andere ziektes kan het aantal CD57 cellen echter verlaagd zijn en er zijn ook chronische Lyme patiënten die wél een normale of zelfs hoge CD57 score hebben. Het is dus niet terecht om de CD57 test een ‘Lyme test’ te noemen. De test heeft beperkte waarde voor Lyme diagnostiek en wordt in Nederland weinig gebruikt..
