The accurate diagnosis of Lyme disease, caused by Borrelia burgdorferi, is critical for effective treatment and prevention of disabling long-term effects. This study investigates discrepancies in serological testing methods for Borrelia infections, focusing on the performance of different assays in annual Lyme screening for high-risk populations. We compared the results of two-tier protocols, including first-tier assays from EUROIMMUN, ZEUS, SERION (all ELISA) and Diasorin (CLIA). Our findings reveal significant variability in test sensitivity: EUROIMMUN combined with ZEUS ELISA detected more positive cases compared to the Diasorin and SERION tests. Notably, 19% of recent Borrelia infections were missed due to test selection, highlighting the clinical implications of these discrepancies. Case studies illustrate the potential for misdiagnosis and delayed treatment, emphasizing the need for standardized and optimized testing protocols. We recommend the adoption of more sensitive and comprehensive methods, and the practice of comparing current results with previous samples from the same individual to improve diagnostic accuracy and patient outcomes..

..The recent VICTORY study evaluated performance of the three cellular tests for borreliosis and demonstrated that they all have lower specificity than the serological tests, which increases the chance of false-positive outcomes (17)..

..In conclusion, our results underscore the importance of serological testing as well as the critical need for standardization and optimization of Lyme disease testing protocols. The use of more comprehensive and sensitive methods, such as the protocol used by Innatoss, should be strongly considered to improve diagnostic accuracy and patient outcomes. Importantly, consensus on the relevance of key antigens such as VlsE should be reached, so interpretation of different immunoblots can be harmonized. Furthermore, the practice of comparing current results to previous samples from the same individual should be more widely implemented in routine serological screening for Lyme disease of high-risk groups..

..Abstract
This retrospective case-control study assesses the sensitivity, specificity, and area under the curve of the ZEUS Borrelia VlsE1/pepC10 assay in comparison with the C6-ELISA in European patients with Lyme borreliosis, healthy blood donors, and potentially cross-reactive controls. We included a convenience series of 161 sera from patients with physician-confirmed early localized or disseminated Lyme borreliosis (n = 143), 400 sera from healthy blood donors and 44 sera with potentially cross-reactive antibodies, on which we performed the aforementioned serological assays and the recomLine immunoblot. Diagnostic parameters were compared in various single-tier and two-tier algorithms. The specificities of the C6-ELISA and the ZEUS Borrelia VlsE1/pepC10 were comparable in healthy blood donors (e.g., single-tier permissive: C6: 362/400, 90.5% [87.2–93.2]; VlsE1/pepC10: 361/400, 90.3% [86.9–93.0]). The C6-ELISA had an apparently higher sensitivity in EM sera (e.g., both time points combined: C6: 61/76, 80.3% [69.5–88.5]; VlsE1/pepC10: 54/76, 71.1% [59.5–80.9]), but these differences were all not-significant. Interestingly, the VlsE1/pepC10 assay had a significantly higher specificity in sera with potentially cross-reactive antibodies (e.g., single-tier permissive: C6: 34/44, 77.3% [62.2–88.5]; VlsE1/pepC10: 40/44, 90.9% [78.3–97.5]; p = 0.031). While the areas under the curve for both assays were excellent, that of the C6-ELISA exceeded that of the VlsE1/pepC10 (C6: AUC = 0.925; VlsE1/pepC10: AUC = 0.878; p = 0.003). The novel ZEUS Borrelia VlsE1/pepC10 assay has generally comparable diagnostic parameters to the C6-ELISA with potentially improved specificity in cross-reactive sera. Thus, it is a useful tool for the serodiagnosis of Lyme borreliosis in Europe..

..In summary, we conclude that the ZEUS Borrelia VlsE1/pepC10 assay has potential to replace the C6-ELISA as a benchmark serological assay for the diagnosis of LB in Europe. We recommend further research to focus specifically on the sensitivity of the assay in early LB and to investigate ways to use the assay in an MTTT algorithm..

..Laboratoria kunnen voor deze metingen kiezen uit verschillende tests van verschillende fabrikanten. Iedere test heeft zijn eigen voor- en nadelen, zoals prijs, gevoeligheid, of de mate van automatisering. Uit wetenschappelijke literatuur weten we dat geen enkele test met kop en schouders boven de andere uitsteekt. Ieder laboratorium maakt zijn eigen keus voor een Lyme antistoftest. Die keuze is afhankelijk van wat dat laboratorium het belangrijkst vindt. Voor Innatoss is de belangrijkste reden om een bepaalde test te kiezen dat de test zo goed mogelijk een recente infectie kan opsporen. Dat, om dit voor elkaar te krijgen, het resultaat bijvoorbeeld wat meer tijd kost, vinden we bij Innatoss acceptabel.

Dit is ook echt belangrijk, want als een test niet goed is in het vinden van een Borrelia infectie, kan iemand onterecht denken dat hij niet besmet is en dus geen Lymeziekte heeft. Daarmee verdwijnt de kans op een tijdige behandeling. Bij een dergelijke test is er sprake van onvoldoende gevoeligheid.

Innatoss houdt elke ontwikkeling rondom Lyme tests in de gaten. Door dit te doen weten we zeker dat we de meest gevoelige tests gebruiken. Om die reden heeft Innatoss een wetenschappelijk onderzoek gedaan waarin een aantal tests met elkaar vergeleken worden. Elena Volokhina, hoofdonderzoeker, legt uit: “We vergeleken de resultaten van veelgebruikte tests en vonden behoorlijke verschillen in de gevoeligheid van de tests. We zagen in het onderzoek dat door minder gevoelige tests 19% (bijna 1 op de 5!) van de besmettingen gemist kunnen worden, wat kan leiden tot onbehandelde Lymeziekte.” Ze zegt met nadruk: “Ons onderzoek laat zien dat gebruik van gevoelige testmethoden, zoals die van Innatoss, van cruciaal belang is om de Lyme diagnose te stellen en patiënten te helpen.”..

..Over onze testmethoden
Alle testen die we gebruiken, zijn CE-gemarkeerd. Dat betekent dat ze aan alle eisen voldoen die binnen de EU gelden voor testen die worden gebruikt voor diagnostiek van Lyme en COVID-19. Wijzigingen in de regelgeving voor dit soort testen volgen we overigens op de voet.

Over onze werkprotocollen
We voeren al onze testen uit volgens een goed gedocumenteerd protocol. Zo zorgen we ervoor dat elk nieuw onderzoek aan onze kwaliteitseisen voldoet. Om een voorbeeld te geven: we gebruiken bij een onderzoek altijd meerdere bloedmonsters ter controle. Die moeten hetzelfde resultaat geven als in de vorige onderzoeken. Als dat een keer niet het geval is, keuren we het hele experiment af, inclusief álle geteste monsters. Dat komt gelukkig niet zo vaak voor, maar het is een goede manier om in de gaten te houden of de uitvoering juist was.

Beoordeling door externe instanties
Om te kijken hoe we presteren in vergelijking met andere labs, doen we elk jaar mee met zogenaamde rondzendingen. Daarbij sturen centrale organisaties alle laboratoria die meedoen een aantal bloedmonsters op. We testen deze en rapporteren wat onze bevindingen zijn. Vervolgens krijgen we te horen wat de resultaten hadden moeten zijn en hoeveel deelnemende laboratoria correcte of verkeerde resultaten hadden ingestuurd. Bij een rondzending voor Lyme rapporteren we dus de resultaten van onze antistoffenmetingen en de eindconclusie of er wel/niet een Borrelia-infectie is vastgesteld. Zo toetsen we achteraf of onze werkwijzen kloppen.

Een rondzending is een belangrijk middel om te bepalen hoe goed onze manier van werken is, van uitvoering tot beoordeling. We leren er bovendien van hoe goed verschillende methoden werken. Voor Lyme doen wij jaarlijks mee aan de rondzendingen van twee grote organisaties: Stichting Kwaliteitsbewaking Medische Laboratoriumdiagnostiek (gevestigd in Nederland) en INSTAND (gevestigd in Duitsland). Wij scoren overigens altijd hoog in beide rondzendingen. Ook doen we inmiddels mee aan rondzendingen voor COVID-19.

Over onze ISO-accreditatie
Voor medische laboratoria is er een speciale internationale kwaliteitsnorm: ISO 15189. Die norm schrijft voor waar we als laboratorium aan moeten voldoen wat betreft personeel, testen, management, inkoop, risicomanagement en nog veel meer. Milly’s body showed a high level of borrelia activity, the bacteria the NHS blood test had missed. She had co-infections of babesia and bartonella, bacteria that are also believed to be spread by ticks and can cause severe illness. These were making her sicker. Her exhausted immune system couldn’t fight them off. Her borrelia infection was tracked with a baseline measurement: 430.50. Only later did Sinclair tell us it was one of the highest he had seen.

@VerlorengezondheidM, hoe verklaar jij dan de resultaten van het wetenschappelijk onderzoek (validatie onderzoek) met de cellulaire testen? Ik ben erg benieuwd naar jouw verklaring! En een punt van aandacht Sinclair is een huisarts/natuurarts en de oprichter van een kleine privé-kliniek en is geen Lyme specialist (infectioloog-internist).

..Onderzoeksresultaten - Subanalyses
Praktische problemen: cellulaire tests zijn foutgevoelig. De gevoeligheid na zes en twaalf weken is niet merkbaar verbeterd. De cellulaire tests presteren vergelijkbaar tussen:
- Patiënten met vroege lyme (erythema migrans) versus gedissemineerde lyme.
- Gezonde personen met een eerdere tekenbeet/Borrelia-antilichamen versus die zonder.

Combinatie van een antistoftest met een cellulaire test maakt deze niet beter.
Bij het testen van lymeziekte is in het algemeen de groep met gezonde personen (zonder lyme) veel groter dan de groep met
lymepatiënten. De onderzoekers stellen dat veel patiënten vals-positieve testresultaten krijgen met de door hen onderzochte testen.

Conclusie
De onderzochte cellulaire tests zijn ongeschikt voor gebruik, vanwege:
- lage gevoeligheid bij zowel vroege als gedissemineerde lyme.
- lage specificiteit..

Met de testen kunnen géén diagnoses Lyme/chronische Lyme/neuroborreliose/Post-lymeziektesyndroomworden en of coinfecties worden gesteld en kunnen géén behandelingen worden gemonitord!!

..Ditzelfde geldt niet voor de cellulaire tests: zij bleken niet betrouwbaar te zijn. Ten eerste hadden ze geen betere gevoeligheid dan de antistoftests waarmee werd vergeleken. Het percentage Lymepatiënten met een positieve uitslag (dus die werden ‘gevangen’) was niet beter dan dat van antistoftests. Veel belangrijker was echter de lagere betrouwbaarheid van de cellulaire tests. Deze tests hadden een aanzienlijk hoger percentage fout-positieve. Er werden dus onacceptabel veel gezonde personen ten onrechte voor Lymepatiënt aangezien. Zij zouden dus op basis van de tests een diagnose kunnen krijgen die ze helemaal niet hadden..

De *CD3/CD57 test mag géén 'Lyme test' worden genoemd en is géén test om chronische Lyme mee vast te kunnen stellen!

Deze test meet via antigenen het aantal CD57 lymfocyten, een bepaald soort bloedcellen die helpen om infecties op te ruimen. Met name chronische Lymepatiënten zouden een laag aantal CD57 cellen hebben en bij antibiotica behandeling en herstel van de ziekte zou dit aantal weer omhooggaan, tenminste volgens sommige ILADS-artsen. Ook bij diverse heel andere ziektes kan het aantal CD57 cellen echter verlaagd zijn en er zijn ook chronische Lyme patiënten die wél een normale of zelfs hoge CD57 score hebben. Het is dus niet terecht om de CD57 test een ‘Lyme test’ te noemen. De test heeft beperkte waarde voor Lyme diagnostiek en wordt in Nederland weinig gebruikt..

..Similarly, Sinclair says 60–70% of his patients make a good recovery with a combination of antibiotics. “I’ve never made the claim that I can cure people,” he says. “But what I try to do is restore function, reduce symptoms and get people into a balance where their immune system should do the work.”

We’ll take those odds with Milly. But our quick-smart daughter has developed a profound scepticism about all treatments after experiencing the failure of NHS doctors and alternative remedies. It took weeks of careful discussion before she would try a combination of antibiotics. We didn’t yet tell her these antibiotics make some patients feel sicker. The Jarisch Herxheimer reaction – diarrhoea, extreme nausea, inflammation – is triggered when the body’s immune system responds to the toxins released as antibiotics start to kill borrelia. Some people can’t tolerate this treatment.

In May she began Sinclair’s protocol of two antibiotics, alongside herbal supplements to support her gut health which would be compromised by the strong drugs..

..Private treatment is beyond the reach of most people. We used cheaper herbal medicines provided via the not-for-profit Claid Clinic but the initial T-spot blood tests cost £2,000. The first three months of treatment, including drugs, supplements, consultations and tests, cost an additional £3,884.50. We are incredibly fortunate to be able to meet these bills – just..

..In August we got her blood test results after three months of antibiotics. We were stunned. Her borrelia levels had dropped from 430.50 to 6. The treatment had obliterated the infection.

Now we are filled with hope, and caution. The numbers still need to reduce to 0.5, says Sinclair, who has tweaked the antibiotic regimen. We continue with herbal medicines, too. School has started again, without Milly, and she remains seriously fatigued. “Sometimes the physical recovery comes later,” Sinclair says. “That’s the hard thing about this. You often reach this period where you go: ‘We’ve done everything, all the figures look good, but the person in front of me is not feeling it.’” Certain types of Milly’s natural killer cells, markers of her immune system health, are still much too low. Sinclair likes a military analogy: “Imagine a battlefield after a war. It takes six to 12 months to clear away the burnt-out tanks. It’ll probably take a while to get back to full function when you’ve removed the enemy.”..

..And what’s the future for Milly? “Because I’ve tried loads of things, it’s hard to believe this will really work,” she says. “I’ve lost faith in medicine and practitioners. I’m not feeling much better either. It’s difficult to think of the future right now – where my life might lead.”..

Het is natuurlijk te hopen dat Milly gaat verbeteren met de kostbare (£7,769-£15,538) gecombineerde antibiotica en supplementen behandelingen maar het kan helaas ook zijn dat zij bij de door de behandelaar genoemde 30%-40% gaat horen die geen baat heeft gekregen bij de behandelingen.
