Hidden in Plain Sight: Chlamydial Gastrointestinal Infection and Its Relevance to Persistence in Human Genital Infection

ABSTRACT

Although the concept of persistence in chlamydial infections has been recognized for about 80 years, there is still very little known about the mechanism by which this occurs. In this review, we revisit an old paradigm, long known to chlamydiologists and veterinarians, that in virtually all hosts of chlamydiae, including mammals and birds, chlamydiae reside in the gastrointestinal tract for long periods of time in the absence of clinical disease. Thus, if gastrointestinal infection occurs in most hosts, then it is very likely that gastrointestinal infection occurs in humans as well. We demonstrate that gastrointestinal infection does indeed occur in humans and propose that this anatomical site is the source of persistent infection in humans. The data in ruminants and animal models demonstrate that the immune system is unable to clear chlamydiae from the gut, so they can remain indefinitely, with continual shedding in feces. Clearly, many women become reinfected from an untreated partner; however, we propose that women, cured of genital infection, remain at risk for autoinoculation from the lower gastrointestinal tract. Moreover, there are substantial data demonstrating treatment failure of chlamydial infections, particularly with azithromycin. New data in the mouse model have shown that azithromycin is far less effective against chlamydial gastrointestinal infection than against genital infections. Therefore, it is possible that women cured of genital infection by antibiotics remain infected in the gastrointestinal tract and can become reinfected by autoinoculation from that site.

IgM was positive only for A. phagocytophilum (512, atypical fluorescence pattern), presenting a low titer of IgG at this stage. Serologic test results for Brucella spp., Chlamydia trachomatis, Chlamydia pneumoniae, and Borrelia burgdorferi were negative.

Bortezomib Eliminates Persistent Chlamydia trachomatis Infection through Rapid and Specific Host Cell Apoptosis

Abstract

Chlamydia trachomatis, a parasitic intracellular bacterium, is a major human pathogen that causes millions of trachoma, sexually transmitted infections, and pneumonia cases worldwide. Previously, peptidomimetic inhibitors consisting of a hydrophobic dipeptide derivative exhibited significant inhibitory effects against chlamydial growth. Based on this finding, this study showed that both bortezomib (BTZ) and ixazomib (IXA), anticancer drugs characterized by proteasome inhibitors, have intensive inhibitory activity against Chlamydia. Both BTZ and IXA consisted of hydrophobic dipeptide derivatives and strongly restricted the growth of Chlamydia (BTZ, IC50 = 24 nM). In contrast, no growth inhibitory effect was observed for other nonintracellular parasitic bacteria, such as Escherichia coli. BTZ and IXA appeared to inhibit chlamydial growth bacteriostatically via electron microscopy. Surprisingly, Chlamydia-infected cells that induced a persistent infection state were selectively eliminated by BTZ treatment, whereas uninfected cells survived. These results strongly suggested the potential of boron compounds based on hydrophobic dipeptides for treating chlamydial infections, including persistent infections, which may be useful for future therapeutic use in chlamydial infectious diseases.

The Magnitude of the STI Epidemic in the United States

In addition to HPV, there are other diseases that are spread primarily through sexual activity. Taken together, they represent a significant public health problem in the United States. According to the latest CDC estimates, more than 65 million people in the country currently live with an incurable STI. Of these, 20 million Americans carry HPV. In addition, researcher Willard Cates estimates that there are 15 million new STI cases in the United States each year. Genital HPV represents one-third of all new STI infections each year. About one-fourth of these infections occur in adolescents. Even though some of the STIs, such as syphilis, are declining, others, like gonorrhea, chlamydia, and genital herpes persist in the population.

Chlamydia is the most common bacterial STI in the United States. If left untreated, 20 percent to 40 percent of women with genital chlamydial infections develop PID, which can lead to infertility, ectopic pregnancy, and chronic pelvic pain. Recent studies have also linked chlamydia with increased risk of developing cervical cancer.
