patients with Lyme carditis often have missed or late diagnoses, which can result in unnecessary pacemaker implantations, complications, and even fatalities. Considering Lyme carditis in the differential diagnosis of young patients presenting with new atrioventricular blocks is critical to ensuring timely diagnosis and treatment.
Secondary POTS means that POTS is associated with another condition known to potentially cause autonomic neuropathy, such as diabetes, Lyme disease, or autoimmune disorders such as lupus or Sjögren’s syndrome.
The Lyme infection causes inflammation of the heart muscle and conduction system. This can result in heart failure due to myopericarditis. It can cause heart block, bradycardia, and the patient may have symptoms of feeling faint or fainting,” she said, adding that simple fatigue may be overlooked as an indicator.
The symptoms of POTS are extensive. “POTS is an autonomic disorder characterized by symptoms such as palpitations, dyspnea, chest discomfort, lightheadedness, nausea, blurred vision, chronic fatigue, sleeping abnormalities, migraines, hypermobile joints, abdominal pain, irritable bowel, and bladder symptoms as well affecting various systems,” writes Tahir.
Autonomic dysfunction has been described in other disorders “such as cerebrovascular disease, movement disorders, dementia, multiple sclerosis (MS) and peripheral neuropathies,”2 explains Xiong et al.
There are a range of pathophysiologic changes in POTS. “POTS has heterogeneous pathophysiology, including excessive sympathetic stimulation, defective peripheral autonomic function, hypovolemia, or autoimmune dysfunction,” writes Tahir.
Symptoms of POTS often overlap with those found in patients with chronic fatigue syndrome and fibromyalgia. Wells and colleagues did not discuss POTS and its association with Lyme disease.
POTS patients have orthostatic tachycardia, which is an increase in heart rate of 30 beats per minute while standing compared to lying down. A small fiber biopsy can be performed in patients to determine if the POTS is neurogenic or caused by a neurologic problem. It has been reported by the Mayo clinic that more than 50% of POTS is caused by small fiber polyneuropathy. Small fibers are involved in autonomic function and if they are damaged, dysregulation of the autonomic nervous system ensues and one manifestation of this is POTS. Bloodwork is used to rule out other conditions that can mimic POTS to identify possible root causes of POTS and to understand if there is an autoimmune component to this condition in specific patients.
Snel herkennen belangrijk
Hartklachten komen vaak voor, daarom is het voor artsen moeilijk om de relatief zeldzame gevallen van ritmestoornissen door de ziekte van Lyme te herkennen. Snelle herkenning is belangrijk om te voorkomen dat de hartritmestoornissen verergeren of zelfs levensbedreigend worden. Ook kan zo voorkomen worden dat het plaatsen van een pacemaker noodzakelijk is.
Bij ongeveer 90% van de gerapporteerde gevallen van lymecarditis gaat het om een hooggradige atrioventriculaire blokkade.4 Het klinische beeld van lymecarditis zien we wereldwijd het vaakst bij mannen, met een man-vrouwratio van 3:1. Over de verdeling bij kinderen is weinig bekend.2 Met de risicoclassificatie Suspicious Index in Lyme Carditis (SILC) kunnen we het risico op lymecarditis schatten
Ernstige vormen van AV-geleidingsstoornissen kunnen leiden tot klachten van verminderde inspanningstolerantie, duizeligheid en syncope of presyncope. Indien er een symptomatisch AV-blok is zonder reversibele oorzaak, dan is implantatie van een pacemaker veelal aangewezen.
Mogelijke ziekteverwekkers zijn virussen zoals het coxsackievirus, influenzavirus (griep), cytomegalovirus (CMV), parvovirus, adenovirus.
Ook bacteriën, zoals mycoplasma, streptokokken en Borrelia (ziekte van Lyme) kunnen een infectie van de hartspier veroorzaken.
Behalve virussen kunnen ook bacteriën myocarditis veroorzaken. De Borrelia burgdorferi-bacterie die de ziekte van Lyme veroorzaakt kan myocarditis ten gevolge hebben. Datzelfde geldt voor sommige parasieten en schimmels.
