Case Report

A 38-year-old Caucasian female presented to our office in January 2011 with chief complaints of left-sided neck, arm and hand pain; left-sided mid-thoracic pain; and left-sided hip pain. She also reported left lower quadrant pain. Her symptoms began 6 years earlier (September 2005) following a therapeutic course of Avelox (moxifloxacin HCl) 500 mg daily for 14 days, for a sinusitis she had developed following a camping trip with her family. While taking this medication for the two-week period, the patient developed a progressively worsening set of symptoms. In addition to the chief complaints described above, she also developed episodic tachycardia, episodic dizziness, episodic shortness of breath, and chronically swollen glands. Additional symptoms included daily episodes of nausea, sweating, tremors, brain fog, blurred vision, panic attacks, and phonophobia.

Discussion

The most commonly discussed side effects of fluoroquinolone medications are those affecting the musculoskeletal system.12 This is important for all clinicians because this family of antibiotics is very commonly prescribed. Back or joint pain may ultimately be aggravated by, if not caused by, these medications.

However, it is important to note that fluoroquinolones were the most commonly prescribed class of antibiotics as of the year 2002.16 The adverse reactions described here previously reportedly occur in only a small minority of cases.2 These may be under-reported, where these symptoms were viewed as separate and distinct symptoms.

Avelox (moxifloxacin) was launched in the United States in 1999 and is marketed in more than 80 countries worldwide. In the United States, Avelox is marketed by Bayer's partner Merck.

In 2011, the FDA added two box warnings for this drug in reference to spontaneous tendon ruptures and the fact that moxifloxacin may cause worsening of myasthenia gravis symptoms, including muscle weakness and life-threatening breathing problems.[6]

Voor wat het waard is, ik had een zeer ernstige neurologische reactie op fluoroquinolones! Zowel cipro- als levofloxacine. Peespijn in achilles binnen een aantal uur na eerste inname (halve dosering zelfs). Ook directe spierzwakte, benauwdheid, wazigheid in hoofd, concentratieproblemen, toename restless legs, pijnovergevoeligheid. En met name levofloxacine gaf me zeer ernstige derealisatie, waarbij ik nauwelijks nog wist waar ik me bevond en hoe ik de objecten die ik voor me zag moest plaatsen in mijn wereldbeeld (heel apart gevoel iig). Ik heb deze middelen beide slechts 2 dagen kunnen nemen alvorens ik er weer mee moest stoppen.

Ik heb een hoop antibioticas en antiparasitaire middelen gehad (denk aan de doxy, mino, clari, azi, rifampicine, flagyl, tinidazole, septra, amoxi, ivermectine, albendazole, mebendazol, pyrantel, nitazoxanide, dec, noem het maar op...) en allen in zeer hoge doseringen, vaak dubbel van wat gebruikelijk is, maar geen van allen gaf de bijwerkingen zoals van de fluoroquinolones! Dus ik denk zeker niet dat dit een herx was door borrelia die-off.

Dat het te maken kan hebben met de NMDA receptor is dus zeker interessant! En kan voor mij wederom wijzen op het grote aandeel dat deze receptor mogelijk heeft bij het in stand houden van de vicieuze cirkel.

Nu ik het boek van Pall aan het lezen ben vroeg ik me al af of er een relatie zou zijn tussen de fluoroquinolones en no-onoo/nmda. Apart dat je dit dan net post
